Change of Spherical Aberration in Early Presbyopia Wearing Center-Near Multifocal Soft Contact Lens

Abstract
Purpose : To investigate the change of spherical aberration (SA) and its effect on the visual acuity (VA) after wearing center-near multifocal contact lens (CN-MFCL) in the early presbyopia.

Methods : The fifty early presbyopic patients were participated. After one week of wearing a CN-MFCL, distance and near VA, SA change and additional power required to improve far and near vision were measured. The visual satisfaction was performed with the questionnaire.

Results : The average refractive and addition power of the patients were -2.65¡¾1.67 D, and +1.15¡¾0.05 D. The ocular SA was 0.01¡¾0.028 ¥ìm, of which 37 subjects (72%) had positive SA (0.028¡¾0.017 ¥ìm), and 13 subjects (28 %) had negative SA (?0.020¡¾0.010 ¥ìm). After CN-MFCL wearing, the SA changed significantly to the negative (-) direction from 0.028¡¾0.017 ¥ìm to -0.012¡¾0.005 ¥ìm in the positive SA group ( <0.050), and the SA significantly increased to the negative direction from ?0.020¡¾0.010 ¥ìm to ?0.043¡¾0.010 ¥ìm in the negative SA group ( <0.050). The 100% distant VA (logMAR) after wearing CN-MFCL was -0.014¡¾0.090 in the positive SA group, and 0.022¡¾0.120 in the negative SA group that shows better distance VA in the positive SA group ( <0.001). The 100% near VA (logMAR) was 0.127¡¾0.090 in the positive SA group, and 0.118¡¾0.090 in the negative SA group that shows better near VA in the negative SA group ( <0.001). In the questionnaire results, the positive SA group showed good distance visual satisfaction, and the negative SA group showed good intermediate and near visual satisfaction ( <0.050).

Conclusion : The SA can be changed after wearing CN-MFCL and visual satisfaction could be changed depending on the contact lens wear¡¯s ocular SA. Therefore, when prescribing MFCL, it is need to consider the wearer¡¯s ocular SA to improve prescription success.
